RESUMO

The National Strategic Plan for the Prevention and Control of Non Communicable Diseases (NSP NCD) outlines the strategic direction for the response to Non Communicable Diseases (NCDs) in Trinidad and Tobago (TTO), and the strategic outcomes that partners from government, private sector, and civil society will be engaged to collaborate towards their achievement over the period 2017-2021. This plan is in alignment with the national development strategy of the Government of the Republic of Trinidad and Tobago (GORTT), which acknowledges that multiple factors and social determinants combine together to affect the health of individuals and communities including inter alia, where people live, work, play, the environment, genetics, income, education, and relationship with friends and family. It utilises key approaches to population health and development including primary health care, universal health coverage, standards of care, integrated management, the Health in All Policies (HiAP), and multisectoral approaches which involve the whole of government (WOG) and whole of society (WOS).

RESUMO

The surveillance of vaccine preventable diseases has been very successful and had been recognised at the highest levels. Trinidad and Tobago was awarded 1st place in 2001 for surveillance of EPI diseases by PAHO/WHO at the Caribbean EPI managers meeting. The ultimate hallmark of a successful immunization programme is demonstrating that vaccine reduces the incidence of diseases therefore routine surveillance must be an important activity in any immunization programme. Surveillance can be termed "watchdog of the EPI". The immunization coverage for all antigens have exceeded 89 percent for all antigens. There is no room for complacency. The levels of immunization coverage must be sustained at over 95 percent so that vaccine preventable diseases will not have any human loss. In addition, let us continue to maintain our efforts to eliminate other diseases such as neonatal tetanus, diptheria, rubella, measles, polio and pertussis. In light of these successes, let us continue to increase our coverage, and prevent missed opportunities at all health facilities, clinics and schools, or wherever vaccination is being administered. New initiatives are needed to reach families in difficult circumstances, areas of severe social privation, mobile families and those in remote areas. It is also important to identify pockets of unimmunized children in urban populations and have them vaccinated. Immunization is a safe and highly effective method of preventing morbidity and mortality due to vaccine preventable diseases and is well established as a most cost effective programme in Public Health

This third edition contains details of important changes and additions. The Immunization Schedule for Trinidad and Tobago now reflects children being immunized as early as possible in life, and incorporates additional vaccines such as Pentavalent vaccine DPT/Hepatitis/Haemophilus influenzae type b.(DPT-Diptheria/Pertussis/Tetanus). The introduction of new combination vaccines improves the ability of the immunization programme to deliver safe and effective vaccines to infants and children, with fewer injections. Another change is that all persons will now receive Adult Diptheria Tetanus (Adult DT) vaccine instead of Tetanus Toxoid (TT) vaccine. MMR vaccine will be given to susceptible post-natal women, and all persons 19-45 years of age (Foreword)

The implementation of quality improvement systems is one of the most interesting and challenging developments in health care; in fact, it has emerged as a central focus for many health delivery systems internationally; Trinidad and Tobago being no exception. Quality systems in the context of health care reform have a fundamental purpose. The objective is to improve and maintain quality, effectiveness and efficiency. Ministries are introducing quality improvement programmes in an attempt to meet the needs of consumers whose expectations and demands have been growing with increasing awareness of their rights. Faced with the dilemma of improving services with limited resources, systems for greater accountability must become an integral part of the quality agenda. External approaches to quality improvement are widely used in many countries and have a long and varied history. These are sometimes described as accreditation, inspection, regulation, audit or external peer review. In Trinidad and Tobago, while the Ministry of Health is playing a pivotal role in establishing the system for accreditation of all health care facilities, the programme will be formally launched under a statutory body. The Ministry of Health however, will retain responsibility for the sytem of licensing of private facilities. The Standards Manual for the Health Sector signals the commencement of a new era in monitoring and regulating health care quality in Trinidad and Tobago (Minister of Health)
